I authorize the release of customer account information by my current provider to eTollFree, LLC, and direct my current provider to release any and all information necessary to complete the porting process.

I understand this authorization remains valid until the port is completed or expressly cancelled in writing. I confirm that all information provided in this form is true and accurate to the best of my knowledge.
